Dental crowns are one of the most common dental procedures. They are used to repair tooth damage and to restore teeth that have been lost or damaged. A dental crown is a cap-like structure that is made of a variety of materials, including gold, porcelain, or a combination of these materials. A dental crown can be used to cover teeth that have been severely worn down by grinding or clenching. It can also be used to replace a missing tooth.
A crown or a cap is used to protect the tooth structure after root canal treatment whereas a bridge is prepared to replace missing teeth. Many patients are unaware that many artificial caps and crowns made of metal or lined with metal have a nickel component. Since precious alloys are used much less these days - most metal caps/crowns/bridges are made with what is called in local slang - 'white metal'. White metal would mean combinations of alloys like cobalt -chromium, nickel chromium and/or others.
Nickel is a metal which causes allergies quite frequently. If you are wearing artificial jewelry causes skin reactions- there is a likelihood that your gum may react the same way (though most gum allergies are relatively more subdued than skin reactions). However there is a way to eliminate the use of nickel and those are 'All ceramic crowns'. These basically are crowns which do not have any metal component whatsoever - much less nickel. This means that the surface closest to your gum will be of the same friendly material as the rest of the tooth.
The next time you visit your dentist - ask about metal free crowns. Do remember when a dentist talks about a 'ceramic crown' he most likely means "METAL CERAMIC'. This means the outer layer is ceramic but the part of the crown that touches your tooth and is closest to your gum is METAL. Many patients walk into the clinic and claim that they have ceramic crowns - a simple examination is enough to tell the difference.
Another common mistake or misunderstanding is with the term - "METAL FREE crowns". Again - metal free does not necessarily mean ALL CERMIC. It may mean Acrylic (PLASTIC) which is neither long lasting nor as friendly to your gum as an all ceramic.

Bad breath is commonly seen in people with poor oral hygiene. Gum disease and dental cavity along with inadequate cleaning of tongue are few reasons to be mentioned for poor oral hygiene. Apart from oral hygiene other factors like acid reflux from stomach, alcohol, smoking, diabetes, eating onion and lots more are reasons to be mentioned for halitosis. Patients usually lose self-confidence during speaking in social gatherings. Bad breath is actually not a disease or major problem. Patients with fixed denture teeth find chewing an easier and cosmetically pleasant smile than those with missing teeth. Now the important point is how patients determine the quality of artificial teeth. First important point is that the patient can feel that the denture teeth should be symmetrical when compared to opposing natural teeth. Some patients complain of teeth big in size when compared to opposite side natural teeth. This happens when dentists do inadequate tooth preparation. Today’s topic is pediatric orthodontist for missing maxillary lateral incisor by Dr Kamana Vijay Rai dentist at Rps dental clinic in Ranchi. Maxillary lateral incisor usually erupts at around 7 to 8 years of age. Some parents are more concerned and visit pediatric orthodontists when milk teeth exfoliate and there is delay in eruption of maxillary lateral incisor. We as a dentist take dental x-ray and match chronology of eruption of permanent teeth. Reasons for delay in eruption of permanent teeth are tooth already present in the path of eruption, insufficient space present for permanent teeth, dental infection, thick gum overlying permanent teeth, ectopic positioning and impacted teeth present in oral cavity. So, teleconsultation alone will not solve the problem. As a dentist we have to evaluate the case clinically and radiographically and of course matches the same with chronology of eruption of permanent teeth. Though many of us know gutka and tobacco chewing habits are bad, still a lot of people are prone to such habits. We as a dentist in Ranchi strongly condemn such bad habits prevalent in society. Teeth got stains both external and internal on long term use of gutka and tobacco. Internal stains are difficult to remove and teeth whitening or bleaching agents are used. External stains are present on the surface of the teeth. Some people brush aggressively and for longer duration in order to remove such stains. But they forget that a toothbrush only removes food particles entrapped between teeth. They don’t whiten teeth or remove internal and external stains. Also, the actual role of toothpaste is to act as a lubricating agent during brushing so that teeth do not wear off easily. Most people think that toothpaste has an actual role during brushing. So being a dentist we treat dental fluorosis to improve patient smile. Teeth appear yellow in colour in dental fluorosis. Patients usually feel uncomfortable as discoloration of teeth can be easily visible during speaking and smiling. So cosmetic dental treatment becomes a necessity to restore a smile. Dentists in our dental clinic perform a number of procedures as cosmetic dental treatment for dental fluorosis. Teeth whitening is the gold standard treatment for discoloration caused by dental fluorosis. Other treatment modality is composite veneering, dental ceramic veneering. Yes, fixed denture teeth are a good treatment option for cosmetic dental treatment. Cosmetically pleasant smile is important for everyone whether young or old age people. Fix denture teeth also known as fixed partial denture is one of the options for improving smile. Other options are partial coverage dental crowns like dental veneers, composite veneering, dental bleaching etc. But the best treatment option as recommended by our dentist is fixed denture teeth. Procedure involved in broader terms refer to smile design. Generally, we take several photographs of the patient and diagnostic casts are of particular importance. Then we evaluate the smile of the patient as recommended by our smile design software. Next, we make a wax pattern on the diagnostic cast. Then comes the second sitting where we actually perform dental treatment after taking consent from the patient. Tooth preparation is a very important step when we perform fixed teeth treatment. Usually, we reduce the tooth crown structure by 2mm. Then we made a dental impression either manually or with a dental intraoral scanner. After that the dental technician makes dental prosthesis. Our dentist does a trial in the patient's mouth and if accepted by the patient we fix the dental crown or dental bridge in the patient's mouth. In spite of so many efforts by our government some people still develop tobacco and gutka chewing habits. Discoloration of teeth in the form of teeth stains can be easily seen. So, to have cosmetically better smile teeth stains in any form have to be removed. Dental stains are of two types and include external and internal stains. Internal stains are difficult to remove and can be done by teeth whitening or dental bleaching procedure. External stains are superficial and can be removed by ultrasonic scaler. This is a professional teeth cleaning method. Single appointment may be sufficient but in some cases two appointments may be required. This treatment can be done by both a general dentist and periodontist. Periodontists are specialized dental doctors for this type of treatment modality.
Cosmetically we can remove gutka and tobacco stains. I have seen cases where a dental patient swears of not eating tobacco after ultrasonic teeth cleaning. Apart from stains in the teeth there are other side effects of eating gutka. To mention a few are limited mouth opening, social stigma during smiling, and last but not the least chances of developing oral cancer or squamous cell carcinoma. Reasons may be multifactorial including eating junk food like chips, sticky chocolates and maintaining poor oral hygiene. Early signs of dental cavity include brown or black discoloration of teeth. Food lodgement between teeth is also common. Dental filling is a very common dental treatment in kids done by kids daant ka doctor or pedodontist. Different types of dental filling material is used in kids but the most common is glass ionomer cement or gic. Reason behind its use is anticavity property. Some suggest use of fluoride application as well to prevent tooth decay.

Today’s topic is infection in the maxillofacial region leading to a visit to a dental hospital in Ranchi by Dr Kamana Vijay Rai. A 20-year female patient came to us. Her chief complaint is pus drainage in the lower right submandibular region. The problem persists for the past 3 to years. She previously visited a dental clinic by searching for a dentist near me online. She had previously visited a skin specialist for the same problem. So, they referred her to our dental center.
Our doctors examined the maxillofacial region and pus drainage is clinically noticed in the lower right submandibular region. Patients have low hemoglobin. Also, ESR is high. She has been taking antibiotics for the past 2 to 3 years to escape from dental treatment like root canal treatment rct or extraction by dental or maxillofacial doctor. Slowly the problem becomes so prominent that she cannot resist visiting us. We advised the patient to undergo a routine blood test and dental x-ray. Root stump is present in lower left first molar teeth.
After getting dental and medical history, examining clinically and radiographically we plan our dental treatment. The treatment as proposed by our dental surgeon and endodontist along with prosthodontist is extraction of lower right first molar and irrigation and drainage of pus which led to tract formation from submandibular region to oral cavity. After completing the surgical step patients have to get antibiotic coverage. Later on, patients can get dental implants or dentures.
